Names and data are separated immediately, if you are still uncomfortable with your name and data being together. Do not put your real name down.
Haven’t done one so I don’t know, but that’s helpful for those who do need to do this.
Census happens every 5 years. The Canadian Community Health Survey and a few of the other core surveys have asked this question in the past.
