The best is Final Fantasy X-2.5 ~Eien no Daishō~, because one of the main plot points is a protagonist tragically dying by mistaking a bomb for a football.
The worst is every Final Fantasy where this does not happen, which I think is all of the others (except maybe Kingdom Hearts??)
